The Hospital Readmissions Reduction Program (HRRP) evaluates readmission rates for six high-impact conditions and procedures: Heart Failure\, Heart Attack\, Chronic Obstructive Pulmonary Disease\, Coronary Artery Bypass Graft Surgery\, Elective Total Hip or Knee Replacement\, and Pneumonia. \nProfessionals play a critical role in ensuring safe\, smooth transitions that reduce the risk of readmissions. This webinar will highlight the most common challenges older adults face after discharge—such as medication management\, mobility limitations\, and follow-up adherence—and provide practical strategies to address them. She began her career working with older adults and Care Professionals at the Omaha Home Instead franchise office and now educates professionals\, families and communities on aging challenges and resources. She is a regular contributor to media outlets including ForbesHealth\, Newsweek\, and hosts Caregiver Chats\, a podcast focused on providing support and resources to family caregivers.
