This practical session helps participants recognize when support is needed, begin planning conversations, and preserve dignity in the final stages of life. While highlighting the role of palliative care, attendees will gain actionable guidance to honor the values, preferences, and goals of individuals facing chronic or end-stage conditions.

Presenter:
David Hage, PhD, MSW, LCSW, FACHE, ACSW, C-ASWCM, CDP, is a gerontologist, a licensed clinical social worker in the states of Florida and Pennsylvania, and an allied health sciences educator currently serving as a member of the faculty in Florida Gulf Coast University’s Social Work Department and as Affiliate Faculty at Shady Rest Institute on Positive Aging.
