National Healthcare Decisions Day is an annual opportunity to educate and empower our communities about the importance of advance care planning.
Advance care planning gives people a voice in their care, and healthcare professionals play a crucial role in this process. By educating your patients or clients about their care options, inquiring about their care goals and priorities, and encouraging them to document their wishes, you can help ensure people receive the care they want and deserve.

My End-of-Life Decisions: An Advance Planning Guide and Toolkit
Use this toolkit to facilitate advance care planning conversations and support individuals in making informed decisions about their care. It covers advance directives, choosing a healthcare proxy, dementia care, and more. Available in English and Spanish, it can be accessed digitally or physical copies can be ordered.
The Dementia Values and Priorities Tool
Use this tool to include dementia in care planning conversations. It invites individuals to reflect on and document their wishes in case of dementia, and generates a dementia directive upon completion. Available in English, Spanish, and Traditional and Simplified Chinese, this tool can be used digitally or in physical form.
