Description:

Individuals, families and communities face complex health challenges such as fractured health delivery systems and different health outcomes. This webinar will provide attendees with practical, evidence-based strategies to assess barriers, engage key stakeholders, and implement collaborative solutions in Age-Friendly Communities. Participants will gain an understanding of core concepts such as social determinants of health, interdisciplinary collaboration, policy considerations, and technology integration to improve health outcomes for older adults. The session will equip attendees with actionable tools to drive sustainable change in their communities.

 

Participants in this webinar will be able to:

  • Identify differences in healthcare access and outcomes related to aging, including barriers such as ageism, fragmented care models, and policy limitations.
  • Examine the intersection of aging and social determinants of health (e.g., housing, transportation, economic stability, and social support) to identify variations in health outcomes.
  • Evaluate evidence-based, upstream strategies for integrating community-based resources with healthcare systems to promote aging-in-place and improve long-term health outcomes.
  • Assess the role of technology in fostering social connectedness, improving healthcare access, and supporting aging-in-community initiatives.
  • Identify and engage in key partnerships that address the needs and opportunities of individuals within the domains of an Age-Friendly ecosystem, including communities, healthcare, public health, employment and universities.

 

Presenters:

Rachel CohenRachel Cohen, MUP, MSW, is the founder and CEO of Aging Dynamics, a consulting firm specializing in aging services and community development. With a background in community planning and social work, she has spent 30 years collaborating with public and private organizations across multiple sectors. Cohen brings extensive knowledge in social services, housing, community development, environmental education, aging and food systems. She helps organizations build strategic partnerships, optimize resources, and align initiatives to create inclusive, equitable, and supportive communities for all generations.

 

 

Erin MaruzellaErin Maruzella, MBA, is an expert in longevity, health equity and age-friendly community development. She advises the tech industry in AgeTech, HealthTech and Age-Friendly Smart Cities, and consults on the development of age-inclusive policies, technologies and community strategies. She serves on the Innovation & Social Impact Advisory Council at ASA, is the vice chair of the Colorado Center for Aging, and is a commissioner for the Pikes Peak Commission on Aging and the Colorado Commission on Aging.
