This webinar will introduce participants to the Return on Investment (ROI) Calculator for Partnerships to Address the Social Determinants of Health, which was developed with support from the Commonwealth Fund. Healthcare organizations and community-based organizations (CBOs) have limited tools to support joint planning, goal-setting and estimating the financial return on investment, sustainability and health outcomes of new pilots or CBO programs that focus on addressing the social needs of complex patients. The ROI Calculator can be an important tool to help organizations collectively address these issues to build better partnerships by helping each partner to become more ready for the work of collaboration.
Participants of this webinar will be able to:
- Understand the context for CBO/healthcare partnerships;
- Understand ROI Calculator tool inputs and process to complete; and,
- Understand the application of this tool in the n4a network.
Presenters:
AJ Diamantopoulos is the Accountable Health Communities manager for the Denver Regional Council of Governments (DRCOG) Area Agency on Aging. He leads the team that works with clinical and community providers to demonstrate the critical link between better health outcomes and increased access to community-based services.
