This webinar will introduce participants to the Return on Investment (ROI) Calculator for Partnerships to Address the Social Determinants of Health, which was developed with support from the Commonwealth Fund. Healthcare organizations and community-based organizations (CBOs) have limited tools to support joint planning, goal-setting and estimating the financial return on investment, sustainability and health outcomes of new pilots or CBO programs that focus on addressing the social needs of complex patients. The ROI Calculator can be an important tool to help organizations collectively address these issues to build better partnerships by helping each partner to become more ready for the work of collaboration.  

Participants of this webinar will be able to:

  • Understand the context for CBO/healthcare partnerships;
  • Understand ROI Calculator tool inputs and process to complete; and,
  • Understand the application of this tool in the n4a network.

Presenters:

AJ Diamantopoulos AJ Diamantopoulos is the Accountable Health Communities manager for the Denver Regional Council of Governments (DRCOG) Area Agency on Aging. He leads the team that works with clinical and community providers to demonstrate the critical link between better health outcomes and increased access to community-based services. Before joining DRCOG in 2013, Diamantopoulos worked as a small business owner and as an advocate for victims of domestic violence, followed by a role as a service coordinator with older adults for Volunteers of America and in Durango, Co. 
Karol Tapias Karol Tapias joined n4a in March 2019 as the deputy director of the Aging and Disability Business Institute. Karol came to n4a from LiveOn NY, where she spent 12 years in multiple roles, most recently as the associate executive director. 
Flynn-Deirdre.png Deirdre Flynn is a director in NFF’s Advisory Services practice and leads engagements with a range of nonprofits and foundations, delivering customized financial management consulting services, including financial analysis and business model assessment, strategic thought partnership and training.
Beth Doreian Beth Doreian is an Associate Director at Nonprofit Finance Fund. As part of the consulting team, she designs and leads customized engagements with nonprofit clients, supporting them through challenge, change and opportunity.
