Goals-of-care conversations can help people living with serious illness clarify their wishes regarding their care, which members of their care team can then use to guide care plans. This may be facilitated by a physician, nurse, social worker, faith leader or another healthcare professional. These discussions are crucial, complex and often challenging. In this session, we will outline important elements of these conversations, review necessary facilitation skills, and learn about the impact they have on patient experience.

Participants in this webinar will be able to:

  • Describe the components of a goals-of-care conversation.
  • Identify a needed skill to facilitate goals-of-care conversations.
  • Outline ways in which these conversations positively impact patient experience.

Presenters:

Jessica EmpeñoJessica Empeño (she/her), MSW, is national director of Clinical Engagement & Education with Compassion & Choices, and a medical social worker with more than 20 years’ experience as a clinician, leader, educator and advocate specializing in end-of-life care, dementia, caregiver support, program development and leadership. In this role, she leads advocacy, education and clinical outreach efforts across the United States to improve care, expand options and empower everyone to chart their end-of-life journey.

 

 

 

Zeena RegisZeena Regis, MDiv, is director of Faith Engagement and Priority Populations at Compassion & Choices. Prior to her role with Compassion & Choices, Zeena served as a hospice chaplain and grief care coordinator for almost a decade. Zeena also serves on the faculty of Columbia Theological Seminary's Older Adult Ministry Certification program.

 

 

 

 

 

Susan WilhoitSusan Wilhoit, MD, is medical director for Compassion & Choices, and a primary care physician, palliative medicine consultant and a hospice clinician, who strives to help patients feel seen and heard. She advocates for their autonomy so they can be empowered to seek the care that best aligns with their values.
